Once your application has been reviewed by the MOH Town Kids Pastor, you will receive an additional email requesting the information needed to authorize a background check. Please open that email and follow the provided instructions to complete your background screening.

I understand that a consumer report (background screening) and/or an investigative consumer report (reference checks and/or interviews) may be obtained in connection with my application to volunteer with MOH SDA Community Mission Church. These reports may include information from public or private sources regarding my character, driving record, criminal history, court records (civil and criminal), qualifications, experience, work habits, and other details relevant to my volunteer service.

I acknowledge that if I am approved to serve as a volunteer with MOH SDA Community Mission Church, this authorization will remain on file and may be used at any time during my service to obtain updated information as deemed necessary by MOH SDA Community Mission Church.

To the extent permitted by law, I release MOH SDA Community Mission Church, its employees, any individual or agency obtaining information on its behalf, and any personal or professional references from all claims, damages, losses, liabilities, costs, or other expenses that may arise from the retrieval, reporting, or disclosure of information during this background investigation.

I understand that I am offering my time as a volunteer and that this role does not make me an employee, subcontractor, or independent contractor of MOH SDA Community Mission Church.

My electronic signature certifies that all information I have provided in connection with this background check is true, accurate, and complete to the best of my knowledge.
